To Re-enroll a Withdrawn Student - If you have previously used FinalForms, please use the 'LOG IN' link below, sign in and choose 're-enroll student.' Complete and sign all forms.

When you enroll or re-enroll your student(s), after all forms are signed you will be contacted for an appointment to provide necessary documentation, such as birth certificate, proof of residency, immunization record, and custody papers, if applicable for your child/children.
