Eaton Community Schools Health and Wellness

Eaton Community Schools is dedicated to the health, wellness, and safety of our students, staff, and community.  In partnership with the Preble County General Health District, we are continually monitoring local, state, national, and worldwide health concerns. 

Meet Our District Nurse
Our District Nurse plays an essential role in supporting the health, safety, and well-being of all students across Eaton Community Schools. She works collaboratively with families, school staff, healthcare providers, and community partners to help ensure that every student is healthy, safe, and ready to learn.

The District Nurse oversees student health services throughout the district, including medication administration procedures, health screenings, immunization compliance, chronic health condition management, and emergency care planning. She also serves as a resource for families regarding illness guidelines, wellness support, and health-related questions or concerns.

We are committed to promoting a healthy school environment and providing compassionate care that supports the success of every Eagle.

Health Forms and Required Documents

We want to make it easy for families to access the health forms needed for enrollment and student care during the school year. The sections below include commonly requested documents for medication authorizations, immunization forms and waivers, and kindergarten physical requirements.

Please complete and submit any forms that apply to your child. Our clinic staff is happy to assist if you need help determining which documents are required.

Student Health Support Resources

To support students with a variety of health needs, we have provided the resources below for families to review and use as needed. These materials include important information, care planning documents, and optional forms related to conditions such as allergies, seizures, concussions, and vision or hearing concerns.

Not all forms will apply to every student. If your child has a specific health need, these resources can help guide next steps and ensure we have the information needed to provide safe and appropriate care during the school day. If you have questions, our clinic staff is happy to assist.
